- THE ORIGINAL SKU STICKER PANEL CUT FROM THE SIDE OF THE HANDSET PACKAGE.
- A REBATE REQUEST FORM COMPLETED IN ITS ENTIRETY. THE REBATE REQUEST FORM MUST INCLUDE THE TEN (10)-DIGIT T-MOBILE WIRELESS NUMBER FOR WHICH YOU ARE REQUESTING THE REBATE, THE FIFTEEN (15)-DIGIT IMEI NUMBER OR FULL MAC ADDRESS FOR THE HANDSET PURCHASED, AND THE DATE OF THE NEW HANDSET PURCHASE.
- YOUR REBATE REQUEST FORM MUST BE POSTMARKED ON OR BEFORE THAT DATE WHICH IS THIRTY (30) DAYS FROM THE END DATE OF THE PROMOTION PERIOD, AS SET FORTH ON THE REBATE REQUEST FORM.
- TO BE ELIGIBLE FOR THE REBATE, YOUR HANDSET MUST BE USED AT LEAST ONCE ON YOUR T-MOBILE ACCOUNT PRIOR TO REBATE VALIDATION.
- YOUR NEWLY PURCHASED HANDSET MUST BE ACTIVATED ON A QUALIFIED T-MOBILE RATE PLAN, AS SPECIFIED ON THE RELEVANT REBATE FORM FOR THE HANDSET OFFER.
- WITH RESPECT TO ANY HANDSET UPGRADE REBATE OFFER, BOTH YOUR CURRENT T-MOBILE SERVICE AGREEMENT AND YOUR NEW T-MOBILE SERVICE AGREEMENT MUST MEET, FOR THE DATES OF THE PROMOTION PERIOD APPLICABLE TO SUCH HANDSET UPGRADE OFFER, THE (A) T-MOBILE TENURE AND (B) RATE PLAN ELIGIBILITY REQUIREMENTS AS SET FORTH ON THE REBATE REQUEST FORM FOR SUCH HANDSET UPGRADE OFFER.

- T-MOBILE IS NOT RESPONSIBLE FOR LOST, LATE, MUTILATED, MISDIRECTED OR POSTAGE DUE REBATE REQUEST SUBMISSIONS. ILLEGIBLE, FRAUDULENT AND INCOMPLETE REBATE REQUEST SUBMISSIONS WILL BE CONSIDERED INVALID AND INELIGIBLE.
