T-Mobile General Rebate Card Terms & Conditions

  1. YOU MUST PURCHASE YOUR HANDSET or ROUTER ALONG WITH A NEW LINE OF SERVICE OR IN CONNECTION WITH AN UPGRADE AND CONTRACT EXTENSION ON A QUALIFIED T-MOBILE RATE PLAN, WITHIN THE PROMOTION DATES SET FORTH ON THE APPROPRIATE REBATE CARD FORM FOR THE HANDSET/ROUTER OFFER. YOUR PURCHASE MUST BE FROM A T-MOBILE RETAIL STORE OR AN AUTHORIZED DEALER.
  2. AS PROOF OF YOUR PURCHASE, AND IN ORDER FOR YOUR REBATE CARD REQUEST TO BE PROCESSED, THE FOLLOWING ITEMS MUST ALL BE SUBMITTED TOGETHER TO THE ADDRESS PRINTED ON THE REBATE CARD REQUEST FORM:
    • A LEGIBLE PHOTOCOPY OF EITHER (A) THE DATED SALES RECEIPT, OR (B) THE T-MOBILE SERVICE AGREEMENT PRINTED WITH THE ACTIVATION DATE, THE IMEI OR T-MOBILE PART NUMBER ASSOCIATED WITH THE HANDSET/ROUTER PURCHASED, AND SELECTED T-MOBILE RATE PLAN (ON WHICH THE HANDSET/ROUTER WAS ACTIVATED) EACH CIRCLED. PHOTOCOPIES OF THE ABOVE DOCUMENTS NOT REQUIRED FOR PURCHASES THROUGH CUSTOMER CARE;
    • THE ORIGINAL SKU STICKER PANEL CUT FROM THE SIDE OF THE HANDSET/ROUTER PACKAGE; AND
    • THE APPROPRIATE REBATE CARD REQUEST FORM COMPLETED IN ITS ENTIRETY.
  3. YOUR REBATE CARD REQUEST FORM MUST BE POSTMARKED ON OR BEFORE THAT DATE WHICH IS THIRTY (30) DAYS FROM THE END DATE OF THE PROMOTION PERIOD, AS SET FORTH ON THE REBATE CARD REQUEST FORM.
  4. TO BE ELIGIBLE FOR THE REBATE CARD, YOUR HANDSET/ROUTER MUST BE USED AT LEAST ONCE ON YOUR T-MOBILE ACCOUNT PRIOR TO REBATE VALIDATION.
  5. YOUR NEWLY PURCHASED HANDSET/ROUTER MUST BE ACTIVATED ON A QUALIFIED T-MOBILE RATE PLAN OR MUST BE ASSOCIATED WITH AN UPGRADE CONTRACT EXTENSION, AS SPECIFIED ON THE RELEVANT REBATE CARD FORM FOR THE HANDSET/ROUTER OFFER.
  6. TO QUALIFY FOR A REBATE CARD WHEN UPGRADING YOUR DEVICE, YOU MUST HAVE COMPLETED AT LEAST 11 ACTIVE MONTHS ON YOUR 1-YEAR T-MOBILE CONTRACT, OR AT LEAST 22 ACTIVE MONTHS ON YOUR 2-YEAR T-MOBILE CONTRACT, AND YOU MUST EXTEND YOUR CONTRACT WHEN UPGRADING YOUR DEVICE.
  7. YOU MUST ACTIVATE A NEW LINE OF SERVICE OR A NEW WIRELESS NUMBER IN ORDER TO BE ELIGIBLE FOR A NEW ACTIVATION REBATE CARD OFFER. ACTIVATION OF YOUR EXISTING WIRELESS NUMBER ON A NEW HANDSET/ROUTER PURCHASED DOES NOT CONSTITUTE A NEW ACTIVATION AND DOES NOT MAKE YOU ELIGIBLE FOR THE REBATE CARD.
  8. WITH RESPECT TO ANY T-MOBILE PAY AS YOU GO HANDSET REBATE CARD OFFERS, YOUR ELIGIBILITY FOR THE REBATE CARD IS SUBJECT TO YOUR ACCOUNT REMAINING ACTIVE ON QUALIFIED T-MOBILE TO GO SERVICE AT THE TIME YOUR REBATE CARD REQUEST IS PROCESSED. IN ADDITION, YOUR T-MOBILE TO GO ACCOUNT MUST BE ACTIVATED (A) WITHIN FOURTEEN (14) DAYS FROM THE NEW HANDSET PURCHASE DATE, AND (B) WITHIN THE ELIGIBILITY PERIOD SET FORTH ON THE APPROPRIATE REBATE CARD FORM.
  9. THERE IS A LIMIT OF ONE REBATE CARD REQUEST PER WIRELESS PHONE NUMBER, IMEI, SMART CARD SERIAL NUMBER, ICCID NUMBER OR ROUTER. THERE IS A MAXIMUM OF FIVE REBATES PER STREET/ CORRESPONDENCE ADDRESS DURING ANY 12 MONTH PERIOD FOR INDIVIDUAL/FAMILY PLAN CUSTOMERS, AND A MAXIMUM OF 100 REBATES PER STREET/ CORRESPONDENCE ADDRESS, DURING ANY 12 MONTH PERIOD FOR BUSINESS/ ENTERPRISE CUSTOMERS.
  10. T-MOBILE PRODUCTS CANNOT BE RETURNED ONCE THE REBATE CARD FULFILLMENT FORM HAS BEEN SUBMITTED.
  11. T-MOBILE IS NOT RESPONSIBLE FOR LOST, LATE, MUTILATED, MISDIRECTED OR POSTAGE DUE REBATE CARD REQUEST SUBMISSIONS. ILLEGIBLE, FRAUDULENT AND INCOMPLETE REBATE CARD REQUEST SUBMISSIONS WILL BE CONSIDERED INVALID AND INELIGIBLE, AND WILL NOT BE PROCESSED.
  12. REBATE CARDS WILL BE MAILED WITHIN EIGHT (8) WEEKS FROM THE DATE YOUR COMPLETED REBATE CARD REQUEST FORM AND ACCOMPANYING MATERIALS ARE RECEIVED.
  13. REBATE CARDS WILL BE MAILED TO YOUR CURRENT T-MOBILE BILLING ADDRESS IN U.S. DOLLARS ONLY. REBATE CARDS NOT USED BY THE EXPIRATION DATE ON THE FRONT OF THE CARD BECOME INVALID AND RELIEVE T-MOBILE OF ANY FURTHER OBLIGATION OR RESPONSIBILITY TO PAY THE REBATE.
  14. DO NOT SEND IN REBATE CARD FORMS OR REBATE CARDS WITH YOUR T-MOBILE BILL FOR PROCESSING.   15. REBATES MAY NOT BE COMBINED OR USED WITH ANY OTHER REBATE, FREE, OR OTHER PROMOTIONAL OFFERS.
  16. REBATE MATERIALS RECEIVED BECOME THE PROPERTY OF T-MOBILE AND WILL BE NEITHER ACKNOWLEDGED NOR RETURNED.
  17. THE REBATE CARD OFFER IS VALID ONLY IN THE U.S. AND PUERTO RICO, AND IS VOID WHERE PROHIBITED, TAXED OR OTHERWISE RESTRICTED BY LAW. TO BE ELIGIBLE FOR THE REBATE CARD, THE INDIVIDUAL SUBMITTING THE REBATE CARD REQUEST FORM MUST BE A LEGAL U.S. RESIDENT WHO IS AT LEAST, 18 YEARS OF AGE AT THE TIME OF SUBMISSION.
